Ann Marie Fazel Age 78, of Rice Lake, WI, died Wednesday, August 10, 2022 in Rice Lake.

She was born on February 15, 1944 in Minnesota to Harold and Marie (Nelson) Lewis.

Ann was a homemaker and a laborer in a manufacturing plant. She was married to Forrest Fazel on June 24, 1983 and they enjoyed many years together.

She is survived by her husband, Forrest Fazel; 3 children, Tina (Shannon) Brady, Christian LeVan and Terra Palmer; 6 grandchildren and several great grandchildren; a sister, Theresa Nemudrov; 2 brothers, Paul (Susan) Lewis and Tom Lewis; many nieces and nephews.

She was preceded in death by her parents; and an infant brother, Christian.

A celebration of her life will be held at a later date.

Appleyard’s Home for Funerals in Rice Lake is assisting the family with arrangements.
