SPOONER, WI -- Spooner Health and Washburn County Health Department are currently conducting a Community Health Needs Assessment. The assessment is a survey that collects data from Washburn County residents and surrounding communities that most likely use Washburn County services to better assess and address health needs.
The community health needs assessment is available online here. You can also pick up a paper copy with a self-addressed stamped envelope at the following locations:

The survey opened on Friday, October 1 and closes on Tuesday, November 30. You must be 18 years of age or older to take the survey and you can only complete one survey. Once the survey is closed, the results are used to create a three-year implementation plan with partnering agencies throughout Washburn County.
