SPOONER, WI -- Spooner Health, Washburn County Public Health, and Indianhead Medical Center are teaming up to conduct a Community Health Needs Assessment. The assessment is a short survey that collects information from Washburn County residents and surrounding communities to better understand and address the health needs of our communities.

The survey period is open from August 1 through October 15. You must be 18 years of age or older to take the survey and you can only complete one survey. Once the survey is closed, the results will be used to create a three-year implementation plan with partnering agencies throughout Washburn County.
